Registration
For registration we do require a valid Driver's License or other valid form of state ID.

New Patients
Any new patients to Florida Medical Doctors Consultants are required to be seen in person for a contactless, covid friendly visit.
Recertification
Patients are required to be seen and recertified every 7 months. This can be done either in person for a contactless, covid friendly visit or with Telemed.
